Attention : Pour la première demande de licence, la visite médicale doit être effectuée par un  médecin  possédant un diplôme de Médecine du Sport

Ajouter : COTISATION A.S.A.C : 40€ délai de 2 semaines minimum

                                     (si délai inférieur à 2 semaines = Majoration de 15€ soit au total 55€)
